I acknowledge that my child may participate in the visual and performing arts, [when applicable] dance exercises, training, instruction, and attendance at onsite/offsite performances and recitals, which comes with the potential for minor and or severe injury. On behalf of my registering child, I assume the risks of participation in the visual and performing arts, exercises, warm-up, training, instruction, and performance events. I approve that the Laity Institute of the Arts may use my registered child’s photograph to promote its business. I certify that my registering child is physically fit for participation in the visual and performing arts, [when applicable] exercises, warm-up, training, instructions, and performance events and has not been advised otherwise by a qualified medical person. I acknowledge that my statements herein are being accepted by The Laity Institute of the Arts and relied upon by any and all sponsors, employees, instructors, organizers, and administrators in allowing my registered child to participate in the visual and performing arts performance [when applicable] dance exercises, training, instruction, events including recitals.
In consideration for allowing my registered child to participate in the visual and performing arts, [when applicable] dance exercises, training, instruction, and performance events, including recitals, I hereby take the following action on behalf of my registered child, child’s executors, administrators, heirs, next of kin and successors and assert:
a) WAIVE, RELEASE, AND DISCHARGE from any and all claims or liabilities for personal injury, sickness of any kind as relating to any COVID-type viruses, cold, flu, or diseases arising naturally in nature, property damage, or damages of any kind which arise out of or relate to the registered child’s participation in the visual and performing arts, [when applicable] dance exercises, training, instruction, attendance at onsite/offsite performances and recitals, which comes with the potential for minor and or severe injury THE FOLLOWING PERSONS OR ENTITIES: Laity Institute of the Arts, its directors, and officers, all event sponsors, instructors, employees, organizers, representatives, and agents of any of the aforementioned
b) AGREE NOT TO SUE any of the aforementioned persons or entities for any claims or liabilities that I have waived
c) APPROVE THE USE OF photographs by the Laity Institute of the Arts to promote its business
d) APPROVE receiving text messages from the Laity Institute of the Arts for information purposes.
e) HEREBY AFFIRM that I am eighteen years of age or older and am acting on behalf of my registered child
f) HEREBY AFFIRM that I have read the Acknowledgement, Waiver, and Release from Liability and Approval of the Use of Photographs and understand its contents
